Sick Leave Letter to Manager Samples

Here are sample sick leave letters. It should be sent by certified mail, so the sender has proof of the time and date it was received. This could be important if there is any legal question pertaining to the proper method of requesting the leave. 

The sender should keep a copy of all communications about the leave application, and any medical letters or documents included should be copies and not original documents. 

Sample 1 – Sick Leave Letter To Manager

Dear Employer’s Name, 

This letter is accompanying my sick leave form to inform you that I am requesting sick leave for three days from DATE to DATE. I will resume work on DATE. I have injured my arm in an automobile accident and have been requested by my doctor to rest in bed. Enclosed is a copy of her instructions. 

I understand that you will employ a substitute teacher to take my sixth grade class for the three days, and I have prepared the lessons plans that cover the classwork and extracurricular activities. I have also included a short description of each student to help the substitute get to know the children.

I have completed the sick leave form required by the school, and according to my contract, I am allowed to take emergency sick leave. 

You or the substitute can reach me at 555-123-4567 or a Name@email.com if there are any questions of if they require any help I can give.

Sample 2 – Sick Leave email To boss

This letter is to inform you that I need to take sick leave starting today. My daughter just called me to inform me that my wife collapsed in the grocery store and has been taken to Name of Hospital. I need to go to the hospital as soon as possible. I don’t as yet know if she is seriously ill, and will inform you as soon as I can.

I plan to return to work the day after tomorrow on DATE because I need a day to arrange for someone to come live in our home for a few days until my wife is discharged from the hospital.

I have informed my colleagues of my situation and am assured that my work will not suffer during this time. I can be reached at Phone Number at any time if you have any questions or would like more information. I can also be reached at Email Address.
