How to Get Medical Records

Last updated on January 12, 2023 / By

There are several ways to obtain your medical records, depending on the circumstances:

1. Contact your healthcare provider’s office directly and request a copy of your records. You may be able to do this in person, by phone, or online, depending on the office’s policies. You may need to fill out a request form and provide identification. There may be a fee for copies of your records.

2. Use the Health Insurance Portability and Accountability Act (HIPAA) Authorization Form to request your records from a healthcare provider. This is a standardized form that allows you to authorize the release of your medical records to yourself or another party.

3. Use an online service to request your records. There are companies that can help you request and obtain copies of your medical records from various healthcare providers.

4. If you are unable to obtain your records from the healthcare provider, you may be able to get them from your health insurance company. Your insurance company may have copies of some or all of your medical records, depending on the type of coverage you have.

It is important to keep in mind that it may take some time to get your records, and the process can vary depending on the healthcare provider and the state you live in.
